Setup and Deployment Package of C# Windows Application with Prerequisites

I developed a c# win application with sql server database and crystal reports.

I need a setup application with all prerequisites i.e .Net framework 4.0 , Crystal Reports for .NET,SQL Server Express.

How to add all prerequisites and how to make setup .

Please guide step wise .
