Create setup (.exe) file of Windows Forms Application with Database in Visual studio

Respected sir,

I have developed windows application on Gym Management using visual studio frameword 4.5 and database using SQL server 2012.

Now i want to create its setup file with database 

i have idea how to create setup file but without database.

please suggest Proper way or video tutorials etc.
