How to display column names along with Sequential number in SQL Server

I have a query but it display only the column name

But i want sequential number alobg with the column name

SELECT COLUMN_NAME
FROM INFORMATION_SCHEMA.COLUMNS
WHERE TABLE_NAME ='tbl_product_desc'
ORDER BY ORDINAL_POSITION

 I want a ouptu lik this
