Restrict user to submit servey form only once in ASP.Net

I want to restrict a user to fill a servery form multiple times.suppose i have an organization and some systems are interconnected using wifi and some are using LAN. But i want if a user Submit Survey form within that organization once.then he or anyother user can not submit the form again.only one user can submit that form he can be anyone else.but if anyone submitted that form then none other person are allowed to submit.if by mistake any person submitted that servery form again with 27 days then both form can be reject on that particular day on which date multiple forms are submitted.
