Download template data from sqlserver database

What is Template here?

I have to design a website where i need one link to be downloaded in pdf format . In that pdf form one text field will be there .Its value must be automatically generated from the database. 

Templte means 

i need it just like

Reg Id:

Name:

Address:

etc.

Sir ,

In the above application values are fetching from database. I need Empty values,because it is just like an application form . The user will print the form and will write thier first name ,lastname,address etc.Only id(unique) is to be printed on the form.
